Important notice

The claim form should be returned to Bank Audi with all documentary evidence to support the claim, within 60 days of the date of the occurrence, loss or event covered by this insurance.
All claims for theft or loss will not be considered unless reported (immediately upon discovery) to the police, or to an appropriate authority where the incident took place and an official written police report is produced.
Failure to complete the claim correctly, sign the form or to provide the required documentation may prejudice or delay your right to indemnity or benefit. The documentation submitted is at your expense and your attention in also drawn to the Excess or Deductibles appearing under this insurance.
